"No one should fall through the cracks of a system that was built to help them. We exist to close that gap — one family, one life at a time."

How we help

Everything your loved one needs,
coordinated in one place

We navigate the healthcare system on your behalf — identifying and coordinating services covered through your insurance, Medicare, Medicaid, and Illinois state programs, so your family doesn't have to figure it out alone.

Case Management & Advocacy

We navigate the complex healthcare and social services system on your behalf — fighting to get your loved one approved for the resources they deserve.

  • Insurance and Medicaid navigation
  • Referral follow-through and coordination
  • Care plan development and monitoring
  • Hospital discharge planning support

DME Coordination

Getting durable medical equipment approved is frustrating and time-consuming. We handle the paperwork, follow up with providers, and make sure your equipment arrives.

  • Wheelchairs and mobility equipment
  • Oxygen and respiratory devices
  • Hospital beds and accessories
  • Provider coordination and appeals

Transportation Coordination

We identify and coordinate transportation benefits available through your insurance, Medicaid, or Medicare — so your loved one never misses an important appointment due to lack of a ride.

  • Medicaid non-emergency medical transport
  • Doctor and specialist appointments
  • Pharmacy and prescription pickup
  • Coordination with approved transport providers

In-Home Support Coordination

We connect your loved one with homemaker and personal care services covered through Illinois Medicaid, the Community Care Program, and other state programs — keeping them safe and independent at home.

  • Illinois Community Care Program navigation
  • Homemaker and personal care referrals
  • Medication reminders and wellness checks
  • Coordination with approved home care agencies
Whole Person Care

Health is more than medicine

What you eat, how you sleep, your stress levels, your social connections — these are not separate from your health. They ARE your health. At SSA we address the whole person, not just the diagnosis.

Nutrition & Diet

What your loved one eats directly impacts their healing, energy, and disease management. We help identify nutrition programs, meal delivery services, and dietary guidance covered through their benefits.

Mental Wellness

Depression, anxiety, and loneliness are among the most overlooked health crises in elderly and disabled individuals. We screen for mental health needs and connect clients to covered behavioral health services.

Social Connection

Loneliness is as dangerous as smoking 15 cigarettes a day. We identify adult day programs, community activities, and social support services that keep your loved one engaged and connected.

Physical Activity

Movement is medicine — even gentle exercise improves balance, reduces fall risk, and boosts mood. We coordinate physical therapy, exercise programs, and community fitness resources appropriate for your loved one's abilities.

Safe Environment

A safe home environment is foundational to health. We assess for fall hazards, coordinate home modifications, and connect clients to programs that fund safety improvements — keeping your loved one safe where they live.

Sleep & Stress

Poor sleep and chronic stress accelerate aging, worsen chronic conditions, and increase hospitalization risk. We address these overlooked factors as part of every comprehensive care plan we develop.

The Social Determinants of Health

Research shows that medical care accounts for only 20% of health outcomes. The other 80% is determined by social, economic, and environmental factors — the things SSA addresses every single day.

Our story

Born from personal experience

"To empower elderly and disabled individuals and their families by navigating the healthcare system on their behalf — ensuring every person receives the care, resources, and dignity they deserve, regardless of how complex the path may be."

— OUR MISSION

A father's struggle became our purpose

When our founder's father became disabled, watching him navigate an overwhelming and often unresponsive system inspired everything SSA stands for today.

Clinically trained, compassionately driven

As a Registered Nurse with a BSN and professional case management experience, our founder brings clinical expertise and personal heart to every client we serve.

Your partner in navigating the system

We understand what resources exist, how to access them, and how to fight when the system says no. That knowledge is now in your corner.

Rooted in healthcare education

Beyond direct care, our founder's background includes training the next generation of healthcare workers — bringing a deeper understanding of the entire care continuum to every client we serve.

When my father became disabled, I watched him struggle to get the help he needed. Referrals went nowhere. Equipment requests were denied. Appointments were missed because transportation was unreliable. As a Registered Nurse with a BSN, I knew the system — and even I found it overwhelming.

That experience planted the seed for Supportive Services Agency. I realized that if I — a healthcare professional — found it this hard to navigate, then families without clinical backgrounds must feel completely lost.

So I built the agency I wish had existed for my father. A place where elderly and disabled individuals are treated with dignity, where families get real answers, and where someone actually follows through until the job is done.

My background spans direct patient care, professional case management, and healthcare education — training the next generation of caregivers. That depth of experience means I understand this system from every angle, and I use that knowledge every day to fight for the people SSA serves.

Today, SSA serves individuals and families across the Chicago area — bringing clinical expertise, genuine compassion, and relentless advocacy to every person we serve.

"Every client we serve is someone's parent, someone's loved one. We treat them exactly that way."

Frequently asked questions

Answers to what families ask us most. Don't see your question? Reach out — we're happy to help.

Supportive Services Agency is a private pay case management and advocacy agency. We charge a coordination fee for our services — please contact us to discuss rates and options. What we do is help you identify and access benefits, equipment, and programs you may already be entitled to through Medicare, Medicaid, and Illinois state programs. Most families find that the value of what we help them unlock far exceeds the cost of working with us.

We primarily serve the Chicago area and surrounding Illinois communities including Cook County and neighboring counties. Many of our services — such as insurance navigation, DME coordination, and care planning — can be provided remotely by phone or video, so reach out even if you're unsure whether we cover your area.

We respond to all inquiries within 24 hours and can typically schedule an initial consultation within a few days. If your situation is urgent — for example, a loved one is being discharged from the hospital — please mention that when you reach out and we will prioritize your case.

Absolutely — a denial is not the end. Medicare and Medicaid denials can be appealed, and many are successfully overturned with proper documentation and advocacy. We review the denial reason, work with the physician to strengthen the clinical documentation, and manage the appeals process on your behalf. Don't give up until you've spoken with us.

A home health agency sends nurses or aides to your home to provide specific clinical services. A case manager like SSA coordinates the bigger picture — identifying what services your loved one needs, navigating insurance and government programs to get those services approved and funded, and making sure everything is working together. We often work alongside home health agencies, not instead of them.

No referral is needed to contact us. Any individual, family member, or caregiver can reach out directly. If a physician referral is needed for specific services we help coordinate, we will guide you through that process as part of our work together.
